Mission

 

The key to the quality of our service is our unique case management system that includes:

  • Communication and liaison with GP's, specialists, hospitals and allied health providers

  • Centralised coordination of care and community resources by Senior Case Managers

  • 24 hour  day telephone support for patients by these same Case Managers

  • Constant communication between Home Nurses, community  nurses and our Case Managers

 

The goal of the Other-Mother Case Management System is to:

 

  • Give doctors a high level of control over the care of their patients

  • Avoid hospitalisation or decrease patients length of stay in hospital

  • Decrease rates of hospital re-admission from a patient’s primary or secondary diagnosis

  • Shorten patient recovery time

  • Refer patient to community resources as appropriate

  • Acute/ Complex Nursing

  • Medication Management

  • Anticoagulant Therapy

  • Acute Mental Health

  • Diabetes Management

  • Continence Management

  • Intravenous Therapy
